益气活血通络法治疗糖尿病周围神经病变52例

摘    要:目的 观察益气活血通络法治疗糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)的疗效。方法 将74例气阴两虚、瘀血阻络型DPN患者随机分为对照组(22例)和治疗组(52例),对照组给予常规疗法,治疗组在对照组疗法基础上加用益气通络活血中药口服,疗程为30 d。观察两组患者神经传导速度、中医证候积分及血液流变学指标的变化。结果 治疗组临床疗效显著优于对照组 (P<0.01),治疗组在提高腓总神经和正中神经的感觉、运动神经传导速度,降低中医证候积分及全血黏度、血浆黏度和纤维蛋白原方面明显优于对照组(P<0.05,或P<0.01)。结论 益气活血通络法对DPN有较好的疗效,其机制与改善血液流变性及感觉和运动神经传导速度有关。

Clinical Effect of Qi-tonifying, Blood-activating, and Collateral-dredging Therapy in Treatment of Diabetic Peripheral Neuropathy: A Report of 52 Cases

Abstract:Objective To observe the clinical effect of qi-tonifying, blood-activating, and collateral-dredging therapy (QBCT) in the treatment of diabetic peripheral neuropathy (DPN). Methods Seventy-four DPN patients with deficiency of both qi and yin and blood stasis obstructing the collaterals were randomly divided into control group (n=22) and treatment group (n= 52). The control group received conventional treatment, while the treatment group received QBCT with oral traditional Chinese medicines (TCMs) in addition to conventional treatment. The course of treatment was 30 d for both groups. The two groups were compared in terms of improvements in nerve conduction velocity, TCM syndrome score, and hemorheological indices. Results The treatment group had significantly better treatment outcomes than the control group (P<0.01). Compared with the control group, the treatment group showed significantly higher increases in the sensory and motor nerve conduction velocities of the common peroneal nerve and median nerve and significantly higher decreases in TCM syndrome score, whole blood viscosity, plasma viscosity, and fibrinogen (P<0.05 or P<0.01). Conclusion QBCT has a good clinical effect in the treatment of DPN, which may be related to the improvements in hemorheological indices and sensory and motor nerve conduction velocities.
