活血通脉颗粒对真性红细胞增多症患者血栓弹力图K值的影响

目的 观察活血通脉颗粒治疗真性红细胞增多症（polycythemia vera，PV）的临床疗效，及其对血栓弹力图K值的影响。方法 随机将60例PV患者分为试验组、对照组各30例。对照组给予西药羟基脲治疗；试验组在对照组的基础上加用活血通脉颗粒。治疗6个月后，观察和比较两组PV患者临床疗效、中医证候评分、外周血象及血栓弹力图K值。结果 两组患者临床疗效分布比较，差异具有统计学意义（P<0.05）；两组患者治疗后证候评分均较治疗前显著降低（P＜0.05），试验组治疗前后中医证候评分差值显著大于对照组（P＜0.05）；两组治疗后血常规各项指标均较治疗前显著降低（P＜0.05），试验组治疗前后血小板计数差值显著大于对照组（P＜0.05）；两组患者治疗后血栓弹力图K值均较治疗前显著升高（P＜0.05），试验组升高至正常值范围，其升高值显著大于对照组（P＜0.05）。结论 活血通脉颗粒可明显改善PV患者中医证候及外周血象，并通过诱导血小板表面受体表达，改善PV患者凝血功能。

Effect of Huoxue Tongmai Granule on K Value in Thromboelastogram in Patients with Polycythemia Vera

Objective To investigate the clinical effect of Huoxue Tongmai Granule in the treatment of polycythemia vera (PV) and its effect on K value in thromboelastogram (TEG). Methods A total of 60 PV patients were randomly divided into experimental group and control group, with 30 patients in each group.The patients in the control group were treated with the Western medicine hydroxyurea, and those in the experimental group were treated with Huoxue Tongmai Granule in addition to hydroxyurea. The clinical outcome, traditional Chinese medicine (TCM) syndrome score, peripheral hemogram, and K value in TEG were observed and compared between the two groups after 6 months of treatment. Results There was a significant difference in the distribution of clinical outcomes between the two groups (P<0.05). Both groups had a significant reduction in TCM syndrome score after treatment (P<0.05), and the experimental group had a significantly greater reduction in TCM syndrome score than the control group (P<0.05). Both groups had significant reductions in routine blood parameters after treatment (P<0.05), and the experimental group had a significantly greater reduction in platelet count than the control group (P<0.05). Both groups had a significant increase in K value in TEG after treatment (P<0.05); the experimental group had a K value within the normal range and a significantly greater increase in K value than the control group (P<0.05). Conclusion Huoxue Tongmai Granule can significantly improve TCM syndrome and peripheral hemogram in PV patients and improve their coagulation function by inducing the expression of platelet surface receptors.
