Carbon monoxide (CO) poisoning as a cause of death is well documented in industrialized countries. The objective of this study was to compare demographic data in deaths due to accidents (in fires) and suicides in the same population between 1988 and 1998. Furthermore, the potential effect of a community wide education effort regarding safety in the home was assessed. Postmortem reports were reviewed for all deaths examined at the Office of the Cuyahoga County Coroner in Cleveland, OH, USA. During the study period, there were 209 accidental deaths due to fires in the home (6.5% of all accidents in the home) and 182 CO deaths by suicide (9.8% of all suicides). Demographic characteristics of the two groups differed: while males represented the majority of cases in both groups (55% of accidents, 70% suicides), race specific death rates were higher for whites than blacks (18/100,000 white, 3/100,000 black) in suicides compared with 29/100,000 deaths for blacks and 11/100,000 for whites in accidental cases. Fire deaths were prevalent in the young (0-9 years) and old (>60) whereas in the suicide group the age specific death rate was highest for those over 70 years. The majority of fire deaths occurred in the city of Cleveland but suicides were prevalent in the suburbs. More fire deaths occurred in December than any other month whereas more suicides occurred in April. In 1992, there was a community wide effort to provide free smoke detectors to residents in Cleveland. In 1992, there were 4.2/100,000 fire deaths in the city. This decreased to 0.6/100,000 in 1996, increased to 1.2/100,000 in 1997 followed by a decrease to 0.8/100,000 in 1998. This suggested that the program may have aided in decreasing these types of deaths. Deaths due to fires in the suburbs were <1/100,000 throughout the study period.  相似文献

This paper examines four propositions inherent in competitive approaches to containing the growth of health care expenditures: (1) that health maintenance organizations can deliver health care less expensively than the fee-for-service system; (2) that under certain competitive conditions, HMOs would prosper; (3) that HMO successes would force FFS insurers and providers to become more efficient; and (4) that creating the competitive conditions would be politically feasible. Reasons for doubting the latter three propositions are plentiful, and the strategy is therefore judged unlikely to succeed.  相似文献

ABSTRACT

Implementing police in schools is a common strategy for ensuring school safety, but it is unknown whether, to what extent, and for whom the presence of police in schools affects student arrest rates. Utilizing nationwide data from the 2013–2014 Civil Rights Data Collection (N = 92,620), this study examines how police presence is related to student arrest rates, and whether this association varies by student race/ethnicity and gender. Path models and propensity score matching models show that the association between police presence and arrest rates was stronger for all the groups examined in schools with police, particularly for Black students and boys. This provides support for criminalization theories suggesting that police presence results in more arrests.  相似文献

The People's Republic of China (PRC) and the USA are, by long measure, the world's largest producers and consumers of coal. Coal production is inherently risky, and fatalities are unavoidable in these large coal economies. Both countries have developed complex systems of law to regulate coal mine safety. These systems share many attributes. Despite similarities between the separate systems of mining law, the PRC significantly trails the USA in terms of coal mine safety. Due to large disparity in economic development, it may be inappropriate to compare these two countries. However, the PRC's mine safety record is significantly worse than that of other large producers who are similarly underdeveloped. It appears that the PRC has failed to effectively implement its mining safety laws. Several arguments have been made as to the cause of this failure, including lack of judicial review of agency actions, lack of meaningful trade unions, government corruption and geographical difficulties of controlling rural mines with a central government. This article explores similarities and differences between the coal economies of the USA and PRC, and introduces some of the arguments used to explain the gap in safety.  相似文献
