小陷胸汤合半夏泻心汤加减治疗痰湿壅盛型老年高血压病23例

目的观察小陷胸汤合半夏泻心汤治疗痰湿壅盛型老年高血压病的临床疗效。方法将46例老年高血压病患者随机分为对照组和治疗组,每组各23例。对照组接受常规西药治疗,治疗组在常规西药治疗的基础上加用小陷胸汤合半夏泻心汤加减治疗,疗程6周。观察并比较两组临床疗效及中医症状积分;治疗前后分别测定两组患者尿微量白蛋白(urinary microalbuminuria,UmAlb)水平。结果两组降压疗效的分布比较,差异无统计学意义(P0.05)。与治疗前比较,两组治疗后各症状积分、中医症状总积分及UmAlb水平显著降低(P0.05);治疗组治疗后头如裹、胸闷、呕吐痰涎评分,中医症状总积分及UmAlb下降值显著大于对照组(P0.05)。结论小陷胸汤合半夏泻心汤加减联合常规西药治疗痰湿壅盛型老年高血压病疗效较好,尤其在改善头如裹、胸闷、呕吐痰涎症状和UmAlb水平方面较常规西药具有明显优势。

Clinical Efficacy of Xiaoxianxiong Decoction Combined with Banxia Xiexin Decoction in Treatment of Elderly Hypertension with Phlegm-dampness Stagnation: An Analysis of 23 Patients

Objective To observe the clinical efficacy of Xiaoxianxiong Decoction combined with Banxia Xiexin Decoction in the treatment of elderly hypertension with phlegm-dampness stagnation. ethods A total of 46 elderly patients with hypertension were randomly divided into control group and treatment group,with 23 patients in each group.The patients in the control group received conventional Western medicine treatment,and those in the treatment group received Xiaoxianxiong Decoction combined with Banxia Xiexin Decoction besides the conventional Western medicine treatment.The course of treatment was 6 weeks for both groups. Clinical outcome and traditional Chinese medicine (TCM) symptom scores were compared between the two groups;the level of urinary microalbumin (UmAlb) was measured for the patients in both groups before and after treatment. Results There was no significant difference in antihypertensive effect between the two groups (P>0.05).The two groups had significantly reduced score for each symptom, total score, and level of UmAlb after treatment (P<0.05). Compared with the control group,the treatment group had significantly larger reductions in the scores for the symptoms of head with binding sensation, chest distress,and vomiting of phlegm-drool, the total score,and the level of UmAlb after treatment (P<0.05).Conclusion Xiaoxianxiong Decoction combined with Banxia Xiexin Decoction and conventional Western medicine treatment has a good therapeutic effect in the treatment of elderly hypertension with phlegm-dampness stagnation, with special advantages in improving the symptoms of head with binding sensation, chest distress, and vomiting of phlegm-drool and the level of UmAlb over the conventional Western medicine.
