A case of diffuse axonal injury (DAI) by assault is reported. The majority of DAI cases documented have been due to traffic accidents and some due to falls from height. DAI is caused by angular or rotational acceleration of the victim's head. The condition is common and is the second most important head injury after subdural hematoma with regard to death. Its clinical picture is characterized by immediate and prolonged coma or demented state. Because of the subtle nature of histological changes in DAI, awareness and intentional search for the lesion is essential. The triad of DAI is as follows: focal lesions (hemorrhages and/or lacerations) in the corpus callosum and brain stem, and microscopic demonstration of axonal damage--retraction balls. The concept of DAI will elucidate and enhance the understanding of many head trauma cases.  相似文献

Several controversies exist regarding ultimately lethal head injuries in small children. Death from short falls, timing of head injury, lucid intervals, presence of diffuse axonal injury (DAI), and subdural hematoma (SDH) as marker of DAI are the most recent controversial topics of debate in this evolving field of study. In this area of debate, we present a case of delayed death from a witnessed fall backwards off a bed in a 9-month-old black male child who struck his head on a concrete floor and was independently witnessed as "healthy" postfall for 72 hours until he was discovered dead in bed. Grandmother, babysitter, and mother all independently corroborated under police investigation that the child "acted and behaved normally" after the fall until death. Autopsy showed a linear nondisplaced parietal skull fracture, diastasis of adjacent occipital suture, subgaleal hemorrhage with evidence of aging, small posterior clotting SDH, marked cerebral edema, and a small tear of the midsuperior body of the corpus callosum consistent with focal axonal injury (FAI). No DAI was seen, and there were no retinal hemorrhages. All other causes of death were excluded upon thorough police and medical examiner investigation. Although this seems to be a rare phenomenon, a delayed, seemingly symptom-free interval can occur between a clinically apparent mild head injury and accidental death in a young child.  相似文献

The main aim of this study was to examine the correlation between the height of fall and the frequency, extensiveness, and type of injuries of certain body regions and organs. The specific objective was to determine characteristic injuries of the analyzed body regions in relation to the certain height of fall. The study included 660 cases of fatal falls from height (469 males and 191 females). Results support the hypothesis that the frequency and extent of the injured body regions and organs are related to the fall height. Head injuries are characteristic of the falls from heights up to 7 m and beyond 30 m. Brain injuries in high falls from heights beyond 30 m show an absence of contre coup contusions and macroscopically evident intracranial bleeding. The extensiveness of chest injuries is significantly statistically associated with fall height. In cases of high falls, the frequency of abdominal injuries is not significantly statistically related to the height of fall. Liver injuries are the most common abdominal injury and the critical height of fall at which the liver injury occurs is 15 m. Injuries of liver and spleen are concomitant in high falls from heights beyond 24 m, irrespective of the manner of impact. The height of fall over 15 m appears to be a reasonable boundary height beyond which the injuries of two or three body regions are generally associated.  相似文献

Falls from great height are suicidal in most cases. Any antecedent trauma that would indicate an involvement of a third party should, however, be excluded in each case. Herein lies the difficulty in such cases since injuries prior to the fall which could be of criminal nature may be masked by the impact injuries. Injuries on unexposed parts of the body should always raise the suspicion of an involvement of a third party. This applies especially for neck injuries. By a retrospective analysis of 132 cases of falls from great height, the authors conclude, however, that neck injuries may occur after free fall from great height on a flat surface without antecedent trauma.  相似文献

According to pediatric statements, falls from a standing position may cause skull fractures in infants without specific symptoms. Nearly every infant has at some time fallen from the changing table or the baby carriage, etc. From the forensic aspect it is the duty of experts to discuss the "battered child syndrome." Evidence given by the defendants concerning the height of the fall are unreliable. Experimental test series concerned with the stumbling height (82 cm in free fall) and three various types of floor-stone, carpet, and foam-backed linoleum -were carried out. In each case skull fractures were seen. In three cases the fractures crossed the sutures. Conclusions: (1) Each fall of an infant from the height of a table may cause a skull fracture, which may lead to death; (2) when child mishandling is suspected, all circumstances must be taken into consideration.  相似文献

Shaken baby syndrome (SBS), one of the most deadly and devastating forms of child abuse, is caused by violent shaking. The combination of subdural hematoma, retinal hemorrhage, brain swelling, and diffuse axonal injury is highly typical of this syndrome and faced with these autopsy findings, induced traumatic lesions are strongly considered. However, it is known that motor-vehicle accidents and falls from great height can also produce this pattern of injury. Nevertheless, stories of arms fall, couch fall, or bumped head while the baby is being carried are generally considered incompatible with SBS. We here report a case of a 2-year-old boy presenting with all the classic autopsy findings of SBS from a playground rocking toy shaken by an older child.  相似文献

The analysis of botanical traces can be supportive, especially when the crime scene investigation and autopsy do not provide sufficient information. This issue arises most commonly in fatal falls from height. We report the case of a man found dead below a hill. The body showed traumatic injuries from a fall. During the scene investigation, a sampling of botanical elements was carried out on three zones of the hill called the point of loss of balance, the point of fall, and the point of impact. The botanical elements collected on the three zones were compared with those found on the victim and an autopsy was performed. The botanical analysis clarified the phases of falling, as well as allowing us to reconstruct the dynamics and the accidental manner of death.  相似文献

The horizontal displacement of the human body resulting from fatal fall from a height is an important variable commonly used to inversely determine the cause or identify other forensic aspects of the fall. When examining the horizontal displacement, the wind effect is generally ignored. This technical note reports analytical modeling of the falling process, utilizing previous measurements of wind force acting on the human body, for determining the functional relationship between the wind speed and the horizontal displacement in the falling process. The result reveals that it does not take extremely rare wind conditions to cause a considerable shift of the human body, highlighting the importance to consider wind as a factor in investigations of fatal falls.  相似文献

Suicides in Geneva in those less than 25 years old, from 1993 to 2002, were reviewed. Scenes investigations, autopsy findings, toxicology results, and psychiatric history (when available) were examined. There were 65 cases. The average annual suicide rate was 11/100,000. Seventy-seven percent were male, and 23% were female. The youngest was 12 years old and most of the victims were 18 years old and over (89%). For men, the use of firearms was the most common method (38%), followed by fall from height (16%) and drowning (10%). For women, fall from height was the most frequent (40%), followed by firearms and medication overdoses (20% each), hanging (13%), and drowning (7%). Toxicological analysis was performed in 41% of the cases and showed that alcohol was present in 26% and other drugs in 67% of these cases. The most common drugs present were benzodiazepines, cannabis, and cocaine.  相似文献

目的研究交通事故致弥漫性轴索损伤(diffuseaxonalinjury,DAI)的患者伤残等级鉴定结果,并探讨DAI对鉴定结果可能产生的影响。方法收集556例本鉴定中心2011年1月-2012年12月受理的交通事故致颅脑损伤案例,其中单纯性脑挫(裂)伤467例,临床诊断为DAI89例。再重新审阅被鉴定人颅脑CT及MRI检查结果,分为单纯DAI组、DAI合并脑挫(裂)伤组、无DAI脑挫(裂)伤组3组颅脑损伤,并对鉴定结果进行比较和统计学分析。结果单纯DAI组(20例)的伤残等级为7.72±1.09,DAI合并脑挫(裂)伤组(69例)的伤残等级为7.78±1.11,无DAI的脑挫(裂)伤组(467例)的伤残等级为8.86±0.66,前两组的伤残等级均高于无DAI的脑挫(裂)伤组(P〈0.05)。结论患有DAI的被鉴定人可能存在更为严重的脑功能损害.鉴定时应注意临床的漏诊和误诊,避免过于依赖CT、MRI等影像学表现。  相似文献

Two case studies are presented involving fatal falls of adult females from a height. One involved a launch at low speed from a balcony, and one involved a launch at high speed from the top of a cliff. Crime scene evidence obtained on the balcony itself provided a strong indication of homicide, but subsequent investigation showed that the fall was accidental. No crime scene evidence was obtained for the cliff fall since the fall initially appeared to be just another suicide from a popular suicide spot. Subsequent investigations indicated homicide based on measurements of cliff height, horizontal distance to the impact, and available runup distance, plus measurements of possible run, jump, and throw speeds. It was found that a female weighing 61 kg (134 lb) can be thrown at speeds up to 4.85 m/s by a strong male, more than enough to account for the estimated launch speed (4.5 m/s). Given the available 4.0 m runup distance, it was found that women of better than average rather than elite athletic ability can dive at speeds of about 3.5 m/s or jump feet first at speeds of about 4.0 m/s, both being less than the estimated launch speed. The decedent had no athletic ability and landed head first after falling through a height of 29 m.  相似文献

目的应用相对和绝对定量同位素标记结合液相色谱-串联质谱法(isobaric tag for relative and absolute quantification-liquid chromatograph-mass spectrometer/mass spectrometer,iTRAQ-LC-MS/MS)筛选SD大鼠弥漫性轴索损伤(diffuse axonal injury,DAI)后脑组织差异表达的蛋白质,寻找诊断DAI的潜在生物标志物。方法参照Marmarou法建立DAI动物模型,分为空白对照组(n=4)、假打击组(n=4)和打击致死组(n=4),采用iTRAQ-LC-MS/MS技术对大鼠脑组织蛋白质进行检测,并对检测结果进行生物信息学分析及验证,筛选出差异表达的蛋白质。结果定量检测出2016种蛋白质,生物信息学分析显示其在细胞中分布广泛、功能多样,并且参与了多种生物过程,16种蛋白质在打击致死组具有差异表达,包括1种表达上调蛋白质和15种下调蛋白质;Western印迹法验证了iTRAQ-LC-MS/MS的结果。结论 iTRAQ-LC-MS/MS的检测技术筛选出大鼠DAI后脑组织中多个差异表达的蛋白质,不仅为深入探讨DAI后轴索损伤的发病机制提供了研究方向,也为DAI的诊断提供了潜在生物标志物。  相似文献

The development and initial psychometric investigation of the Dating Attitudes Inventory (DAI) is reported. The DAI was created, to fill a gap in the literature and to measure specific masculine ideology and traditional gender attitudes that rationalize the abuse of women. Using a sample (n = 164) of male college students, a 20-item measure was developed consisting of two subscales (Rationalization of Abuse and Dominance and Control) and a total score. The 20-item DAI and other measures used for validation were completed by 216 male college students. The DAI correlated in theoretically expected ways with measures of propensity for abusiveness, relational dominance, and masculine gender role stress. A confirmatory factor analysis supported the two theorized factors of the DAI. Results of the present study offer initial support for the validity and reliability of the DAI. The authors discuss the importance of measuring masculine gender role attitudes and beliefs that support and rationalize dating violence.  相似文献

The major issues of medico-legal relevance in fatal falls from a height are the manner of death and the reconstruction of the event. We present a peculiar case of a fatal fall from a height of about 9 m, involving a 27-year-old woman. At the death scene investigation, no suicide notes, housebreaking marks, or signs of fight were found, thus weakening both the suicide and homicide hypotheses. Combining circumstantial, autopsy and toxicology data, the kinematic analysis of the jump/fall, and the histological evidence of a myocardial sarcoidosis involving the left ventricle, we hypothesized that the young woman might have accidentally fallen from the window because of a sudden loss of consciousness related to cardiac disease undiagnosed during life. We believe that our brief report is a good example of the powerful additional information that histological investigations can offer for reconstructing the dynamics of the event in falls from a height and other traumatic fatalities.  相似文献

目的对高坠案件进行统计分析,为相关鉴定研究提供参考。方法收集上海市浦东地区最近十年来记录有坠落距离的246例高坠案件,对人体损伤的特点进行比较分析。结果自杀高坠的高度显著高于意外高坠的高度(25.70m:13.98m);所有高坠中,头部损伤概率较高(75.2%),颈部损伤概率较低(6.5%);意外高坠的头部损伤概率高于自杀组(89.4%:75.2%)。结论意外高坠的死者比自杀高坠的死者更缺乏四肢对于头部的保护;过低的颈部损伤发现率提示国内法医工作者应该更加重视在高坠案件中的对于颈部损伤的检查。  相似文献

弥漫性轴索损伤(diffuse axonal injury,DAI)以脑白质轴索弥漫性损伤为主要特征,迄今其形成机制尚未完全阐明,不同学科诊断标准迥异,是法医学和神经科学亟待解决的难题。磁共振弥散张量成像(diffusion tensor imaging,DTI)通过检测弥散参数及纤维束成像(fibre tracking,FT)可准确地反映轴索损伤部位及损伤程度,具有广泛的法医学应用前景。本文综述DAI发生机制及诊断方法的研究进展,尤其DTI技术在DAI诊断中的应用。  相似文献

目的探讨利用傅里叶红外光谱技术诊断弥漫性轴索损伤的可行性。方法利用HE染色、银染和β-APP免疫组化染色确认大鼠弥漫性轴索损伤模型,利用傅里叶变换显微红外光谱面扫描成像技术检测弥漫性轴索损伤区域酰胺Ⅱ带的光谱分布情况,得到弥漫性轴索损伤的红外光谱数据,绘制红外光谱病理图像。结果实验组与对照组的酰胺Ⅱ带红外光谱吸收度之间存在明显差异,红外光谱病理图像中酰胺Ⅱ带的高吸收区与弥漫性轴索损伤区符合。结论傅里叶红外光谱技术可对弥漫性轴索损伤进行病理形态学诊断。  相似文献

More than two decades ago, Marmarou published a valid model for producing diffuse axonal injury (DAI) in rats. Since then, both mild and severe injuries have been obtained by researchers using the original method and a weight of 450 g. However, the diffuse brain injuries produced in rats were only similar to those seen in humans when the rats sustained severe brain injuries. In these cases, rat mortality in the original article was around 50%, and the cause of death was prolonged apnea post-impact. Rat survival after impact is critical for studying the progression of DAI. In order to explain the cause of death in human victims with cranial trauma who do not show gross brain injury, testing for the presence of DAI is essential. Thus, in order to minimize local and cervical injuries to increase rat survival, attention should be paid to the following aspects: a wider head protector disc should be used, the head of the rat should be elevated at the time of impact, and the foam bed should be soft enough to allow the movement caused by acceleration. With our modified method, rat survival increased by 30% compared to the original model (80% versus 50%). Moreover, 85.7% of rats demonstrated DAI after 24 h of survival. With these modifications, injuries appear in the same locations as in humans; thus, the method is suitable for the study of traumatic DAI in humans.  相似文献

While it is recognized that veterans have increased rates of depression, post‐traumatic stress disorder (PTSD), suicide, and substance use disorders, rates of homicide and unintentional injury deaths in veterans have been minimally investigated. We evaluated all non‐natural deaths in New Mexico veterans between 2002 and 2011 in comparison with non‐natural deaths among non‐veterans. We reviewed all decedents in New Mexico with a history of military service and investigated by the medical examiner, excluding natural deaths and deaths due to fall from standing height. The most common manner of death was unintentional injury (62%), most of these deaths due to motor vehicle accidents (29%) followed by unintentional overdose (26%). Suicide rates among veterans were consistently higher than the general population. The most common mechanism of suicide in men was gunshot wound (72%), and intentional overdose in women (49%). Services are needed for veterans that are tailored to all ages and both sexes.  相似文献

The objective of this study was to evaluate the application of Fourier transform infrared (FTIR) spectroscopy for detecting diffuse axonal injury (DAI) in a mouse model. Brain tissues from DAI mouse model were prepared with H&E, silver, and β‐amyloid precursor protein (β–APP) immunohistochemistry stains and were also studied with FTIR. The infrared spectrum images showed high absorption of amide II in the subcortical white matter of the experimental mouse brain, while there was no obvious expression of amide II in the control mouse brain. The areas with high absorption of amide II were in the same distribution as the DAI region confirmed by the silver and β‐APP studies. The result suggests that high absorption of amide II correlates with axonal injury. The use of FTIR imaging allows the biochemical changes associated with DAI pathologies to be detected in the tissues, thus providing an important adjunct method to the current conventional pathological diagnostic techniques.  相似文献
